We are two Registered Nurses with more than 10 years of experience working with the pediatric population. We specialize in Case Management and coordination of care for medically fragile and special needs children.
Our purpose is to support the entire family not just the patient because our experience has taught us the importance of communication and collaboration amongst all individuals involved in the care of the child. Therefore, we will provide care management for families to have one place where all individuals caring for the child come together, we will work with PCP, pharmacies, specialists, therapists, providers, DME companies and parents to create one team supporting the needs of the family to achieve the child’s goals.

THE PROCESS
The care coordination process includes identifying providers, assessing needs, planning care, implementing the care plan and then evaluating options and services to meet the child's and family's individual needs.
